Redefining Foreheads: Advanced Frontal Bone Reshaping and Hairline Lowering Techniques By Dr. Ralph Abbou

Updated: Apr 11

As the field of facial aesthetics becomes increasingly precise, forehead contouring and frontal hairline harmonization are now essential pillars of surgical beautification. Once limited to reconstructive indications or niche feminization procedures, these techniques have evolved into mainstream aesthetic solutions for both men and women seeking proportion, balance, and youthfulness.

I’ll be presenting these innovations in detail at AIME Congress 2025 in my session: June 19 | 11:00 AM – 11:45 AM

I/ Frontal Bone Reshaping: From Cranioplasty to Aesthetic Precision

What it is:Frontal bone reshaping consists of sculpting the frontal convexity (forehead and supraorbital region), often to reduce prominence or achieve a more feminine, smooth curve.

Latest surgical refinements:

  • Piezoelectric bone sculpting, reducing thermal trauma and improving precision over rotary instruments

  • Minimally invasive endoscopic approaches now emerging for selected cases

  • Short scar to minimize scarring and achieve a natural hairline

Case example:

A 29-year-old female patient presented with prominent brow ridges and forehead bossing. Using a tailored supraorbital reduction and orbital rim remodeling, we restored balance in his upper facial third—improving both frontal aesthetics and confidence. Recovery was complete in under 3 weeks.

Clinical advantage:

Compared to older burring-only or camouflaging techniques, today’s approach ensures:

  • Symmetry and finesse, especially in female patients

  • Reduced surgical time and bleeding thanks to new tools

  • Durability with minimal need for revisions

II/ Hairline Lowering: Surgical vs. Grafting

Indication: High forehead, long upper face ratio, gender-affirming needs, or post-transplant correction.

Modern technique steps:

  • Trichophytic hairline incision (scar concealed by hair regrowth)

  • Wide scalp undermining to mobilize the frontal flap with minimal tension through a minimal scar

  • Optional frontal bone reshaping combined in one procedure

Key benefits over traditional methods: Immediate, measurable lowering (20–30mm possible) More predictable and much more efficient than hair grafting, especially in patients with fine or thin frontal hair Often combined with brow lift, providing rejuvenation and harmonization in one surgery

Post-op outcome:

  • Social downtime: 10 days

  • Scarring: Virtually invisible at 3 months

  • Complications: Minimal with proper vascular preservation

Growing global demand:This technique is rapidly gaining traction in the US, South Korea, and Brazil, driven by high expectations for facial harmony in beauty-focused cultures. France is emerging as a key player thanks to its fusion of aesthetic artistry and surgical innovation.

III/ Who Are the Ideal Patients?

These procedures are now applied far beyond feminization or reconstructive indications.

Examples of ideal candidates:

  • Women with genetically high foreheads seeking a more youthful facial ratio

  • Men with frontal bossing and wide orbital rim looking for subtle masculinization

  • Patients with prior hair transplants, dissatisfied with density or hairline shape

  • Middle-aged patients combining it with an upper face lift or blepharoplasty

V/ See It Live at AIME Congress 2025

Join me on Thursday, June 19 from 11:00 AM to 11:45 AM for the “Surgical Facial Bone Beautification” session.

Co-chaired with Prof. Mohammad Sartawi (Jordan), we will explore the technical nuances, live case insights, and practical implementation of these techniques in everyday aesthetic surgery.

Whether you are a facial plastic surgeon, aesthetic doctor, or maxillofacial specialist, this session will give you new tools to refine your approach and expand your offer in a competitive and evolving market.
